Employee Referral Award Form

To refer a potential employee, please complete this form to the best of your ability.

Submit the Employee Referral Award Form using the button below.

This form must be returned either before or during the hiring process. The form will not be accepted after the employee is hired.

You, the employee, are eligible for a $500.00 referral award only when you refer external candidates who become employees.

The employee must remain employed by Hyde Concrete, full-time, without incident, and in good standing for a minimum of six (6) months.

Only one referral award can be given per candidate. If a candidate is referred by more than one employee, the first referral received will be the one rewarded.

Solo se puede otorgar un premio de recomendación por candidato. Si un candidato es referido por más de un empleado, la primera referencia recibida será la premiada.
